This nomination form is for horses of all breed registries other than those registered with Canadian Warmblood Horse Breeders Association, which are automatically nominated upon registration and are invoiced annually. However, this form may be used to manually nominate CWHBA horses which have been registered late.

* Herd Rate - fees are capped at five horses for horses born in the same year. Please indicate each horse on a separate Nomination Form.

Signed submission of this form, is:

• Confirmation that that the owner(s) of the horse has read the rules of the Canadian Performance Futurity and agrees to abide by them.
• Agreement by the owner(s) of this horse to hold CWHBA and its executive, directors, employees, and volunteers harmless to any injury, including physical or financial, that participation in this futurity may incur.
• Agreement that, in the event of change of ownership or change of address, the owner(s) will inform the Canadian Performance Futurity prior to the next scheduled payment.
